- Take with food if trying it. Evidence is insufficient despite widespread claims. Systematic reviews of magnesium for subjective anxiety conclude the existing evidence is of poor quality - trials are small, use heterogeneous populations and outcome measures, frequently combine magnesium with vitamin B6 or other nutrients making attribution impossible, and several are at high risk of bias. Where effects appear, they are small and concentrated in people with low magnesium status, mild anxiety or premenstrual symptoms, and reviewers have consistently called for adequately powered trials that have not yet been done. The mechanism - GABA-A modulation, NMDA antagonism, reduced HPA reactivity - is sound, which is why the hypothesis persists, but mechanism is not evidence of clinical effect.

Magnesium is effective for mild-to-moderate depression in adults. It works quickly and is well tolerated without the need for close monitoring for toxicity.
Magnesium Status and Stress: The Vicious Circle Concept Revisited

Stress increases urinary magnesium excretion and depletes magnesium stores, while magnesium deficiency amplifies HPA axis activation and stress hormone release - a self-reinforcing vicious circle. Mechanistically, magnesium antagonises NMDA receptors, supports GABAergic transmission and dampens cortisol release. Supplementation trials, particularly magnesium combined with vitamin B6, show reduced perceived stress scores in individuals with severe stress and low magnesium status.

Caveats
Oxide form is poorly absorbed. Most benefit is in those with inadequate intake.
