    Sleep maintenance - staying asleep rather than falling asleep - is a distinct problem from sleep onset, and it is frequently driven by nocturnal cortisol and a hyperactive stress response. That is precisely the physiology ashwagandha is proposed to act on.
    The classic pattern is waking at three or four in the morning with the mind immediately active. Where that reflects stress-driven arousal rather than pain, apnoea or alcohol, an agent that lowers cortisol and supports GABAergic tone is a reasonable thing to try. The botanical name is suggestive rather than evidential - somnifera means sleep-inducing - and the honest framing is that ashwagandha addresses the stress physiology underlying fragmented sleep rather than acting as a hypnotic.

    How It Works

    Withanolides, the steroidal lactones concentrated in the root, modulate the hypothalamic-pituitary-adrenal axis. Lower nocturnal cortisol means less of the arousal signal that fragments the second half of the night.
    A second route is GABAergic: preclinical work reports withanolide activity at GABA-A receptor signalling, the same broad target as prescription hypnotics though far weaker. Triethylene glycol, an isolate from the leaf, has also been proposed as a sleep-inducing component in animal work. Both routes favour continuity of sleep rather than speed of onset, which fits the maintenance framing: less cortical arousal during the night, fewer awakenings, easier return to sleep after one.

    Fix the obvious causes first

    Alcohol, late caffeine, a warm bedroom and irregular bed times fragment sleep more reliably than any supplement can repair.

    The broader ashwagandha sleep literature consists of small randomised trials, usually 50 to 150 participants over 6 to 12 weeks, mostly reporting questionnaire outcomes such as the Pittsburgh Sleep Quality Index, with a minority adding actigraphy. Sleep efficiency and subjective quality are the endpoints most often reported, and sleep-onset latency more often than maintenance specifically. Wake after sleep onset - the direct measure of maintenance - is inconsistently captured, so this pairing rests partly on inference from cortisol physiology.

    Safety

    Evening ashwagandha is generally well tolerated. Morning grogginess, vivid dreams and gastrointestinal upset are the most common reports, and grogginess usually responds to a lower dose.

    Rare liver injury has been reported

    Case reports describe hepatotoxicity with ashwagandha, prompting regulatory warnings in several countries. Stop and seek medical advice if jaundice, dark urine or persistent abdominal pain occurs.

    It is contraindicated in pregnancy, with caution in autoimmune disease, thyroid disease and hormone-sensitive prostate cancer. Loud snoring with witnessed pauses, gasping awakenings or unrefreshing sleep despite adequate hours point to sleep apnoea, which needs investigation rather than a supplement.
