Overview
The brain reorganises most readily in the first three to six months after a stroke, which is why rehabilitation intensity during that window has an outsized effect on final function. Repetition matters more than novelty: constraint-induced movement therapy, task-specific practice and high-dose gait training all outperform general exercise. Recovery continues beyond six months, and the older advice that improvement stops at that point is wrong and discouraging. Alongside rehabilitation, secondary prevention determines whether a second stroke undoes the gains — blood pressure control is the single largest lever, followed by antiplatelet or anticoagulation therapy, lipid lowering, and atrial fibrillation detection. Supplements have been tested here and failed: homocysteine-lowering B vitamin regimens in VITATOPS and VISP reduced homocysteine substantially without preventing recurrent stroke. Malnutrition and dysphagia are the nutritional issues that genuinely affect outcomes, and both need active screening.

When to See a Doctor
Any new stroke symptoms — face droop, arm weakness, speech difficulty — require emergency care immediately, even if they resolve. Arrange review for worsening swallow, unexplained weight loss, or low mood during recovery.
