    Effect of B-vitamins on stroke risk among individuals with vascular disease who are not on antiplatelets: A meta-analysis

    Methodology

    Systematic review and meta-analysis of randomized homocysteine-lowering trials (VISP, VITATOPS, HOPE-2) stratified by antiplatelet use, follow-up >= 1 year.

    Key Findings

    Among 4,643 high vascular risk subjects not taking antiplatelet medication, B-vitamin supplementation was associated with reduced subsequent stroke risk; benefit was not seen in those on antiplatelets.

    Conclusions

    Any cerebrovascular benefit of homocysteine lowering appears limited to patients not receiving antiplatelet therapy.

    Limitations

    Only three trials reported antiplatelet-stratified data; post-hoc subgroup analysis prone to confounding.
