Overview

Roughly a quarter of people progress to type 2 diabetes within five years without intervention, but the same window makes it one of the most reversible conditions in medicine. The Diabetes Prevention Program remains the defining evidence: intensive lifestyle intervention aiming at 7% weight loss and 150 minutes of weekly activity reduced progression by 58%, compared with 31% for metformin, and the effect persisted in long-term follow-up. Weight loss was the strongest single predictor, with each kilogram lost associated with roughly a 16% reduction in risk. Two mechanisms deserve emphasis because they change behaviour. Skeletal muscle takes up glucose independently of insulin during contraction, which is why a 10-15 minute walk after meals blunts post-meal glucose measurably and why resistance training improves insulin sensitivity through increased muscle mass. Sleep is the underrated variable: even a few nights of restricted sleep reduce insulin sensitivity substantially in controlled studies. Supplement evidence is modest against that backdrop. Berberine has trial data approaching metformin in some studies but with quality limitations, magnesium helps where intake is low, and chromium has largely failed in well-run trials.
