Overview
Elevated blood pressure describes readings between 120-129 systolic with a normal diastolic, sitting between optimal and full hypertension. It matters because risk is continuous: cardiovascular risk begins rising well below the diagnostic cut-off, and most people in this range progress to hypertension within a few years if nothing changes. The upside is that this is the stage where lifestyle change has its largest proportional effect. Weight loss, sodium reduction, the DASH eating pattern, alcohol moderation and regular aerobic exercise each lower systolic pressure by a few millimetres of mercury, and they stack. Accurate measurement matters more than people expect — a properly sized cuff, a seated rest of five minutes, back supported and feet flat, arm at heart level, and an average of readings across several days at home. Single clinic readings routinely overstate pressure, and treating an artefact helps nobody.

Root Causes
Excess body weight and visceral adiposity drive sympathetic activation and sodium retention. High dietary sodium with low potassium shifts fluid balance and vascular tone. Alcohol raises pressure dose-dependently above roughly one to two drinks daily. Physical inactivity, poor sleep and untreated obstructive sleep apnoea all contribute, as does chronic psychological stress through sustained sympathetic drive. Arterial stiffening with age raises systolic pressure independently of all of these.

- Correctly sized cuff and validated device — the single biggest source of error
- Ambulatory 24-hour monitoring where white-coat or masked hypertension is suspected

- Renal function, electrolytes and urine albumin-to-creatinine ratio
- Thyroid function where clinically indicated

When to See a Doctor
Book a review if averaged home readings stay at or above 130/80, if you have diabetes or kidney disease at any elevated reading, or if pressure keeps climbing despite lifestyle change. Seek urgent care for readings above 180/120, and emergency care if that is accompanied by chest pain, breathlessness, severe headache, visual change, weakness or confusion.

Potassium

Suggested Pattern
The DASH pattern remains the best-evidenced diet for blood pressure, lowering systolic pressure by around 6 mmHg on its own and by roughly 11 mmHg when combined with sodium restriction — comparable to a single antihypertensive drug. It emphasises vegetables, fruit, low-fat dairy, whole grains, nuts and legumes while limiting red meat, sweets and saturated fat. Potassium is the quiet workhorse: most people eat far too little, and increasing it helps counterbalance sodium. A Mediterranean pattern performs similarly and is easier for many people to sustain.

- Potassium-rich foods: leafy greens, potatoes with skin, beans, bananas and avocado
- Beetroot and other nitrate-rich vegetables, which modestly lower pressure
- Oily fish two to three times weekly
- Unsalted nuts, seeds and legumes
- Low-fat dairy, a distinctive component of the DASH pattern
- Whole grains in place of refined starches
Avoid
- Processed and packaged foods, which supply most dietary sodium — not the salt shaker
- Cured meats, tinned soups, sauces and takeaway meals
- Alcohol above one drink daily for women or two for men
- Liquorice root in quantity, which raises pressure directly
- Decongestants containing pseudoephedrine and regular NSAID use
