    Garlic is one of the few botanicals with a reproducible blood pressure effect. Meta-analyses of randomised trials, including an updated analysis pooling around 970 participants, report systolic reductions of roughly 8 mmHg and diastolic reductions of around 5 mmHg in people who start out hypertensive. The qualifier is important: the effect is concentrated in people with elevated readings. In normotensive participants the change is small to negligible, which is reassuring rather than disappointing.
    An 8 mmHg systolic shift is comparable to a single low-dose antihypertensive at a population level, but individual response varies widely and garlic has not been tested against cardiovascular endpoints. It belongs alongside sodium reduction and exercise as part of a lifestyle-first approach, not as a replacement for prescribed medication.

    How It Works

    The active chemistry runs through organosulfur compounds. Allicin, formed when raw garlic is crushed, is unstable and rapidly converts to a family of sulfides, while aged garlic extract instead concentrates stable water-soluble compounds such as S-allylcysteine.
    These compounds serve as hydrogen sulfide donors. Hydrogen sulfide is a gasotransmitter that relaxes vascular smooth muscle, and garlic-derived sulfides also increase nitric oxide availability and reduce oxidative inactivation of it. The net effect is lower peripheral resistance. Secondary effects include mild inhibition of angiotensin-converting enzyme activity, reduced platelet aggregation, and modest improvements in arterial stiffness measures.     Preparation is the variable that most often explains disappointing results. Aged garlic extract has the cleanest trial record and the least odour; standardised powdered tablets should specify allicin yield rather than raw garlic weight. Deodorised products that have destroyed the sulfur chemistry are unlikely to work.

    Safety

    The common problems are social rather than medical: breath and body odour, plus reflux or a burning sensation with raw preparations on an empty stomach. Taking supplements with food and choosing aged garlic extract reduces both. Rare allergic reactions and contact dermatitis from handling raw cloves are reported.

    Stop before surgery

    Garlic has antiplatelet activity. Discontinue supplemental doses at least 7 to 10 days before surgery or dental extraction, and take medical advice before combining it with anticoagulants or antiplatelet drugs. If you are already on blood pressure medication, monitor for excessive lowering rather than stopping your prescription.
