    Grape Seed Extract for Elevated Blood Pressure

    Grape seed extract lowers systolic blood pressure by roughly 6 mmHg in pooled randomised trials, with the effect concentrated in younger adults and those with metabolic syndrome rather than in older established hypertensives.

    Grape seed extract is rich in oligomeric proanthocyanidins, and its blood pressure effect is real but smaller than garlic's. Meta-analysis of randomised trials of cardiovascular risk markers found modest systolic reductions, with the clearest signal in younger participants, people with metabolic syndrome, and those with obesity rather than in older established hypertensives. Typical reported reductions are in the range of 3 to 6 mmHg systolic over 8 to 16 weeks.
    That is a worthwhile lifestyle-tier effect, particularly for someone in the pre-hypertensive band who wants to avoid medication, but it is not sufficient for stage 2 hypertension. Extract standardisation varies enormously between products, which is a large part of why trial results are inconsistent.

    How It Works

    Proanthocyanidins improve endothelium-dependent vasodilation. They upregulate endothelial nitric oxide synthase and, as strong antioxidants, reduce superoxide-driven inactivation of nitric oxide, so more of the nitric oxide produced actually reaches vascular smooth muscle.
    There is a secondary, weaker angiotensin-converting enzyme inhibitory action, and evidence of reduced vascular inflammation and improved flow-mediated dilation on ultrasound. This mechanism profile predicts what the trials show: better response where endothelial dysfunction is the dominant problem — metabolic syndrome, obesity, prehypertension — and less benefit where arterial stiffening from long-standing hypertension has already set in.

    Dosing & Protocol

    Look for standardisation to proanthocyanidin or polyphenol content rather than raw extract weight. Doses used in trials cluster between 150 and 300 mg daily; higher doses have not shown proportionally greater benefit, and some trials used as little as 100 mg with effect.

    Safety

    Grape seed extract is generally well tolerated at 150 to 300 mg daily. Reported effects are mild: headache, nausea, dry mouth or a scratchy throat, usually early and transient. Rare allergic reactions occur in people sensitive to grapes.

    Bleeding risk and iron absorption

    Proanthocyanidins have mild antiplatelet activity — stop at least 2 weeks before surgery and take advice before combining with anticoagulants. They also bind non-haem iron, so if you are managing iron deficiency, separate doses by at least 2 hours from iron-rich meals or supplements. Safety in pregnancy and breastfeeding is not established.
