    Take with ample water and separate from medication by 2 hours. Evidence is mixed. Psyllium is only partially fermentable - roughly 30-50% is fermented in the colon while the rest retains water and provides bulk, which is precisely why it works for both constipation and diarrhoea but makes it a weaker prebiotic than inulin, fructo-oligosaccharides or resistant starch. Studies show psyllium increases short-chain fatty acid production modestly and shifts some bacterial populations, with one trial showing effects that differed between constipated and healthy participants. Its main benefits - LDL reduction, glycaemic control, stool normalisation - are largely mechanical and osmotic rather than microbiome-mediated.

    What the studies say

    Trials show increases in butyrate-producing taxa in some populations, particularly people with constipation, but shifts are smaller and less consistent than with classic prebiotics.

    If the goal is specifically feeding gut bacteria, psyllium is not the optimal choice - inulin, fructo-oligosaccharides, galacto-oligosaccharides, resistant starch and a varied whole-food diet with legumes, whole grains, vegetables and fruit are more fermentable, though they also produce more gas. Psyllium is better regarded as a bulking and viscous fibre with excellent evidence for cholesterol, glycaemia and stool regulation. Dietary plant diversity remains the intervention with the best evidence for microbiome composition, and consumer stool tests are not validated for guiding fibre choice. Safety ceilings: fluid is essential - insufficient water has caused oesophageal and intestinal obstruction, so at least 250 mL per dose, never at bedtime, and avoid with swallowing difficulty, oesophageal stricture or obstruction history. Separate all medication by at least 2 hours, particularly levothyroxine, lithium, carbamazepine, digoxin, warfarin and iron. Psyllium lowers blood glucose, so monitor if taking insulin or sulfonylureas. Bloating and gas are common initially. Rare allergic reactions including anaphylaxis are documented. In pregnancy, psyllium is generally considered safe. Persistent gut symptoms with weight loss, bleeding or anaemia need investigation rather than fibre.
