    Psyllium Husk for Cardiovascular Risk Reduction

    Psyllium is one of few supplements with an FDA-authorized heart-disease-risk-reduction claim. Meta-analyses confirm ~5–10% LDL reduction at ~10 g/day, additive to diet. Effect is modest but well-established and safe.

    Overview

    Psyllium is unusual among supplements in having a regulator-authorised heart health claim behind it. The FDA permits the statement that soluble fibre from psyllium husk, as part of a diet low in saturated fat and cholesterol, may reduce the risk of coronary heart disease. That claim rests on a consistent, well-replicated effect: at around 10 g per day psyllium lowers LDL cholesterol by roughly 6 to 10%, on top of whatever diet or medication is already in place.
    The honest caveat is that the endpoint here is inferred rather than measured. No randomised trial has run psyllium against heart attacks or cardiovascular deaths as a primary outcome. What exists is a reliable improvement in the risk factors that drive those events - LDL, blood pressure, glycaemic control - plus large cohort data linking soluble fibre intake to fewer coronary events. That is a reasonable chain of reasoning, but it is a chain, not a single trial. Treat psyllium as an additive, low-risk contribution to cardiovascular risk management rather than an alternative to statin therapy where statins are indicated.

    How It Works

    Psyllium husk is a viscous soluble fibre. In the small intestine it forms a gel that traps bile acids and prevents their reabsorption in the terminal ileum. The liver must then synthesise replacement bile acids from cholesterol, and it does so by upregulating hepatic LDL receptors to pull more cholesterol out of circulation. The result is a lower serum LDL concentration.
    The same viscosity slows gastric emptying and blunts post-meal glucose excursions, which explains the modest improvements in fasting glucose and HbA1c seen in people with impaired glucose tolerance. Fermentation of a portion of the fibre by colonic bacteria produces short-chain fatty acids, which may contribute a further small effect on hepatic lipid handling. Because the mechanism depends entirely on gel formation, adequate water intake is not optional - it is part of the dose.

    Dosing & Protocol

    Start at 3 to 5 g in at least 250 mL of water once daily with a meal, and build up by 3 to 5 g every three or four days to 10 to 15 g per day in divided doses. Escalating too quickly is the main cause of bloating and cramping, and the main reason people abandon it. Lipid changes appear at four to eight weeks, so recheck a lipid panel at around eight to twelve weeks rather than sooner.

    Safety

    Psyllium is one of the better-tolerated interventions in this space. Bloating, flatulence and a feeling of fullness are common in the first week or two and usually settle as the dose is built gradually. Adequate fluid is essential: taken dry or with too little water, psyllium can cause oesophageal or intestinal obstruction, and it should be avoided entirely by anyone with a known bowel stricture, swallowing difficulty or suspected obstruction.

    Always take psyllium with a full glass of water and never immediately before lying down. Do not use it if you have difficulty swallowing or a history of bowel obstruction.
