    Overview

    This is one of the most firmly established effects of fish oil. A meta-analysis of randomised trials found omega-3 supplementation significantly reduced platelet aggregation, with larger effects at higher EPA and DHA doses.
    A clear dose-response relationship across randomised trials is about as solid as supplement pharmacology gets. It also explains observations going back to studies of populations with high marine fat intake and long bleeding times. The framing worth holding onto is that this is a genuine pharmacological effect, which cuts both ways. It is the basis of the cardiovascular rationale, and it is also why anyone on anticoagulants or approaching surgery needs to treat fish oil as an active drug rather than a benign nutrient.

    How It Works

    Platelet activation runs largely on thromboxane A2, produced from arachidonic acid in the platelet membrane. EPA displaces arachidonic acid from that membrane and is metabolised instead to thromboxane A3, which is far less effective at aggregating platelets.
    At the same time, endothelial cells produce more prostacyclin I3, which opposes aggregation. So the balance shifts on both sides of the equation: less potent activation signal from the platelet, more inhibitory signal from the vessel wall. This is substrate competition, which is exactly why the effect is dose-dependent. The more EPA that occupies membrane phospholipids, the less arachidonic acid is available for thromboxane A2, and the larger the reduction in aggregation.
