- Evidence is insufficient. Nattokinase is a fibrinolytic serine protease, and its documented action is on fibrin and on the fibrinolytic system through plasminogen activator inhibitor-1 - which is a different pathway from platelet aggregation. Platelets aggregate via ADP, thromboxane A2 and glycoprotein IIb/IIIa mechanisms, which is what aspirin and clopidogrel target. A few small studies at 2,000-4,000 FU/day have examined platelet or clotting parameters with inconsistent results, and no adequately powered randomised trial has demonstrated a reliable reduction in platelet aggregation in humans. So the fibrinolytic activity is real, but attributing an antiplatelet effect to nattokinase goes beyond the data - while the bleeding risk from combining it with real antiplatelet drugs is not theoretical.
