    Evidence is insufficient. Venous return is determined by blood volume, venous tone, the skeletal muscle pump, respiratory pump, valve competence and right atrial pressure - none of which nattokinase has been shown to affect. Its documented activity is fibrinolytic: degrading fibrin and modulating plasminogen activator inhibitor-1. Small trials have measured blood pressure, fibrinogen and clotting factors, and one small study examined leg oedema during prolonged sitting, but no randomised trial has measured venous return, venous tone or venous insufficiency outcomes. The idea seems to originate from conflating thrombosis prevention with venous function, which are separate problems - and even for thrombosis, no clinical endpoint trial exists.

    What the studies say

    Small trials show reduced fibrinogen and some improvement in leg oedema during long flights. Evidence is preliminary and largely from one research group.

    No studies are yet linked to both Nattokinase and Venous Return.

    Bleeding risk, particularly with anticoagulants. Avoid before surgery.
