Amyloid burden itself has not been shown to fall.

    Caveats

    Amyloid is not something you can track, and the amyloid hypothesis itself is contested - anti-amyloid antibody drugs clear plaque substantially while producing only modest clinical slowing, which suggests plaque reduction alone is not sufficient. For dementia risk reduction, the interventions with real evidence are blood pressure control in midlife, physical activity, not smoking, treating hearing loss, managing diabetes, limiting alcohol, education and social engagement, and treating depression. Progressive memory loss affecting daily function needs medical assessment, since reversible causes including B12 deficiency, hypothyroidism, depression, sleep apnoea and medication effects are common. Safety of omega-3: fishy aftertaste, reflux and loose stools are common. Doses above 3,000 mg/day EPA+DHA increase bleeding risk - use caution with warfarin, direct oral anticoagulants, aspirin and antiplatelet drugs, particularly relevant in older adults, and stop before surgery. Several trials found an increased incidence of atrial fibrillation with high-dose omega-3. Avoid fish oil in fish or shellfish allergy; use algal oil. Avoid high-dose cod liver oil, since vitamin A retinol above 3,000 mcg/day is toxic and teratogenic. In pregnancy, choose low-mercury sources and avoid high-vitamin-A liver oils.
