    Take with meals. Evidence is mixed but not negligible. Several small randomised trials, mostly conducted in India in people with type 2 diabetes or metabolic syndrome, report reductions in fasting and post-prandial glucose with holy basil leaf extract, with effect sizes in the range of 0.5-1.5 mmol/L for fasting glucose, and a systematic review concluded the direction of effect is favourable. The limitations are substantial: trials are small, often poorly blinded, use widely differing preparations and doses, and few report HbA1c. This is worth a monitored trial as an adjunct, not a replacement for established treatment.

    A randomised crossover trial in type 2 diabetes reported reduced fasting and postprandial glucose with tulsi leaf powder, and later reviews found consistent direction but low-quality evidence.

    Safety

    Caveats

    Do not stop or reduce prescribed diabetes medication to use holy basil - uncontrolled diabetes causes irreversible eye, kidney and nerve damage, and any medication change must come from your prescriber. Safety ceiling and interactions: holy basil lowers blood glucose, so combining it with insulin or sulfonylureas risks hypoglycaemia - monitor closely and discuss dose adjustment with your clinician. It has antiplatelet activity and may increase bleeding risk with warfarin, DOACs, aspirin and clopidogrel; stop two weeks before surgery. Animal studies suggest possible antifertility and anti-spermatogenic effects with long-term high doses, and holy basil may have uterine-stimulant activity - avoid in pregnancy, when trying to conceive, and while breastfeeding. It may interact additively with pentobarbital and other sedatives. There is no established upper intake level and limited long-term human safety data.
