    A cheap, testable adjunct for post-prandial glucose. Small effect on longer-term markers, and no substitute for carbohydrate quality or medication.

    How It Works

    Acetic acid delays gastric emptying, spreading carbohydrate delivery into the small intestine and flattening the glucose curve. It also inhibits sucrase and maltase, slowing the liberation of absorbable monosaccharides, and enhances muscle glucose uptake through AMPK activation in preclinical work. Because these actions are local and time-limited, the effect only applies to the meal it accompanies.

    Evidence

    What the studies say

    Small crossover studies consistently show that 10-30 ml of vinegar taken with a starchy meal reduces the post-prandial glucose excursion by 20-30% in healthy, insulin-resistant and type 2 diabetic participants, with a corresponding reduction in the insulin response. Effects are largest with high glycaemic index meals such as white bread or rice, and negligible with low-carbohydrate meals. Longer trials are fewer and smaller: meta-analyses of 8-12 week studies in type 2 diabetes suggest reductions in fasting glucose of around 0.3-0.5 mmol/L and HbA1c changes of roughly 0.2-0.5 percentage points, but the pooled trials are small, heterogeneous and mostly unblinded, so confidence is limited. Vinegar taken at bedtime has been reported to lower fasting morning glucose in a couple of studies, which is intriguing but not well replicated. There is no evidence for any glycaemic effect when vinegar is taken away from food. The practical significance is that this is a marginal-gains intervention: worth doing, verifiable with a home glucose meter, and not remotely competitive with metformin or with reducing refined carbohydrate.

    Safety

    Caveats

    It only works with the meal. Undiluted vinegar erodes tooth enamel and has caused oesophageal injury. It is contraindicated in gastroparesis, where slowing gastric emptying is precisely the wrong action, and it can worsen reflux. Chronic high intake has been associated with hypokalaemia in case reports. Gummies frequently contain added sugar and unverified acetic acid content.

    Less likely to help if

    People eating low-carbohydrate meals, where there is little glucose excursion to blunt, and those with gastroparesis or significant reflux who cannot tolerate it. It will not compensate for a diet high in refined carbohydrate.
