    Apple cider vinegar is the rare supplement where the modest real effect and the marketing claims live in entirely different universes. The real effect is post-prandial glycaemia: taking roughly 20 ml of vinegar with or just before a carbohydrate meal reduces the subsequent glucose peak, typically by 20-30%, an effect replicated across small crossover studies in healthy, insulin-resistant and type 2 diabetic participants. The mechanism is straightforward — acetic acid slows gastric emptying and inhibits disaccharidase activity, so glucose enters the bloodstream more gradually. Small trials also show modest fasting glucose and HbA1c improvements over 8-12 weeks, though these studies are few and small. Then there is everything else. Weight loss trials show differences of one to two kilograms over twelve weeks, within the range easily explained by unblinded expectation and appetite suppression from nausea. Detoxification, immune support, pH balancing and blood pressure claims have no meaningful clinical basis. The safety issues are not trivial either: undiluted vinegar erodes tooth enamel and has caused oesophageal injury, and chronic high intake has been linked to hypokalaemia in case reports. Diluted, taken with meals, through a straw, it is a cheap and reasonable glycaemic adjunct — nothing more.
