    Panax quinquefolius contains the same broad ginsenoside family as Asian ginseng but a different profile — higher Rb1, lower Rg1 — which is the pharmacological basis for its reputation as calming rather than stimulating. Two clinical claims survive scrutiny. First, glycaemic control: repeated crossover trials show 3 g taken up to two hours before a carbohydrate load reduces post-prandial glucose in both healthy and type 2 diabetic participants, with the effect dependent on timing relative to the meal. Second, fatigue: a multi-site randomised trial in cancer survivors found 2,000 mg/day of ground root significantly improved fatigue scores by week eight with no added toxicity, which is a rare positive in that literature. Cognitive data are more mixed — acute working memory improvements have been reported but not consistently replicated — and the immune claims rest largely on the COLD-fX proprietary polysaccharide extract, which reduced cold frequency in older adults in company-linked trials. Quality is the recurring practical problem: ginsenoside content varies enormously and adulteration with cheaper Panax species is well documented.

    Potential Side Effects

    Generally mild. Headache, digestive upset and, less often than with Asian ginseng, insomnia or nervousness. Hypoglycaemia is possible when combined with glucose-lowering medication.

    Contraindications

    Warfarin therapy, hormone-sensitive cancers, pregnancy and lactation, uncontrolled hypoglycaemia risk.
