Separate fenugreek from other medicines by two hours, since the fibre reduces their absorption.

    Caveats

    Fenugreek adds to the effect of insulin, sulfonylureas and other glucose-lowering medication, so hypoglycaemia is a genuine risk - monitor blood glucose closely when starting and do not adjust prescribed medication without medical advice. Never replace diabetes medication with fenugreek. Safety ceiling and interactions: it has anticoagulant properties and should be avoided with warfarin, DOACs, aspirin and clopidogrel, and stopped two weeks before surgery. The soluble fibre reduces absorption of other oral drugs - separate by two hours. Contraindicated in pregnancy: fenugreek has uterine-stimulant activity, is a traditional labour inducer, and has been associated with maple-syrup body odour in newborns and possible birth defects in animal studies. It is in the legume family, so avoid with peanut or chickpea allergy. Expect a maple-syrup smell to sweat and urine, plus wind, bloating and diarrhoea. Anyone with symptoms of very high glucose - excessive thirst, heavy urination, weight loss, blurred vision - needs medical review, not a supplement.

    Less likely to help if

    People with type 1 diabetes, those with advanced beta-cell failure needing insulin, and anyone unable to tolerate the gastrointestinal effects at an effective dose.
