- Evidence is insufficient for testosterone support as a clinical goal. Trials of 500-600 mg/day standardised fenugreek extract report small changes in free testosterone in some studies and none in others, with total testosterone frequently unchanged, and the trials are small, short and largely manufacturer-funded. None enrolled men with diagnosed hypogonadism, none measured clinical endpoints such as bone density, muscle mass or fertility, and none showed a change large enough to matter clinically. Confirmed hypogonadism is treated with medically supervised testosterone replacement.
