Its phytochemistry is dominated by eugenol, ursolic acid, rosmarinic acid and ocimumosides, and the pharmacology that emerges is genuinely adaptogenic in the technical sense: it normalises the hypothalamic-pituitary-adrenal response to stress rather than simply sedating or stimulating.
The human evidence is best for psychological stress. Randomised trials of 500-1200 mg/day of leaf extract over six to eight weeks report reductions of roughly 30-39% in stress symptom scores, with improvements in sleep quality, forgetfulness and sexual problems as secondary outcomes. Anxiety scores fall in a similar range. These are small, mostly Indian trials with heterogeneous extracts, so the effect size should be read cautiously — but the direction is consistent and the placebo-controlled design is present in most of them.
Metabolically, holy basil has a surprisingly solid signal. Trials in type 2 diabetes show fasting glucose reductions of roughly 17-27 mg/dL and postprandial reductions of a similar magnitude, likely via improved insulin secretion and reduced hepatic gluconeogenesis. Because of that, anyone on glucose-lowering medication should monitor for hypoglycaemia. The other practical caution is antiplatelet activity from eugenol — stop it two weeks before surgery — and animal data suggesting reduced fertility, which makes it a poor choice for couples trying to conceive.
