    CoQ10 for Enhanced Athletic Performance

    CoQ10 is essential to mitochondrial ATP production, which makes it an obvious candidate for an exercise supplement and a persistent disappointment as one. Muscle CoQ10 concentrations in healthy, well-fed people are already near saturation, and oral supplementation raises plasma levels far more than it raises muscle levels. The result is a literature of small, inconsistent effects: some trials report improved time to exhaustion or reduced perceived exertion, many report nothing measurable at all.
    Where CoQ10 looks more useful is recovery rather than raw output. Several trials report lower markers of exercise-induced muscle damage and oxidative stress after heavy training, which may matter more for people training frequently than for a single performance test. Statin users and older adults — groups with genuinely lowered CoQ10 status — are the populations most likely to notice anything.

    How It Works

    CoQ10 shuttles electrons from complexes I and II to complex III in the electron transport chain, so every ATP molecule produced aerobically depends on it. It also functions as the only lipid-soluble antioxidant the body synthesises, protecting mitochondrial membranes from the reactive oxygen species that intense exercise generates in quantity.
    The gap between mechanism and result is a delivery problem. CoQ10 is a large, highly lipophilic molecule with absorption in the low single-digit percentages, and skeletal muscle uptake from plasma is slow and tightly regulated. Weeks of dosing raise muscle content only modestly in people who are not deficient. This explains the pattern in the trials: acute dosing does essentially nothing, two-week protocols do a little, and the clearest responders are people whose baseline status was low to begin with.

    Dosing & Protocol

    Take CoQ10 with a fat-containing meal — absorption on an empty stomach is poor enough to waste the dose. Ubiquinol or a solubilised ubiquinone formulation is worth the premium; plain powder-in-capsule ubiquinone is the least well absorbed form.

    Evidence

    The linked trial in the Journal of the International Society of Sports Nutrition compared acute and 14-day CoQ10 supplementation in both trained and untrained participants, which is the right design for this question because it separates the two claims usually made for the supplement. The acute arm showed no meaningful benefit, consistent with the pharmacokinetics. The 14-day arm produced higher muscle CoQ10 and some improvement in exercise measures, with the untrained group generally showing more room to move than the trained group. The magnitudes are small, and this remains a supplement with a stronger recovery rationale than a performance one.

    Safety

    CoQ10 has an excellent tolerability record across trials running months to years at doses up to 1200 mg daily. Reported effects are mild and infrequent: nausea, loose stools, appetite loss and occasional insomnia if taken late in the day.

    Warfarin and blood pressure

    CoQ10 is structurally similar to vitamin K and has been reported to reduce warfarin's effect — do not start it on warfarin without INR monitoring. It can also modestly lower blood pressure, which matters if you are already on antihypertensives. Safety data in pregnancy and breastfeeding are limited, and there is no need to time doses around training since the effect depends on accumulated tissue levels.
