    Caveats

    Heart failure requires prescribed treatment - never substitute or delay it. Loop diuretics deplete thiamine, which is the only plausible mechanism here. Thiamine has no established toxic ceiling orally, but parenteral forms carry rare anaphylaxis risk. Safe in pregnancy at normal doses.

    People with normal thiamine status, and those with heart failure not on diuretics.
