    Thiamine pyrophosphate is a cofactor for pyruvate dehydrogenase and transketolase, enzymes central to neuronal glucose oxidation and myelin maintenance.

    Deficiency causes Wernicke-Korsakoff syndrome, which responds to parenteral thiamine. Cochrane reviews find no reliable cognitive benefit of thiamine in Alzheimer disease or in replete adults.

    Caveats

    Wernicke-Korsakoff syndrome is a medical emergency - oral supplements are inadequate. No cognitive benefit has been shown in replete adults. Thiamine is water-soluble with no established toxic ceiling, but high-dose parenteral forms carry rare anaphylaxis risk and belong in clinical settings. Safe in pregnancy at normal doses, and often needed in hyperemesis gravidarum under medical care.

    Less likely to help if

    People with normal thiamine status and no deficiency risk, and those with established Korsakoff amnesia where damage is fixed.
