    Mixed evidence

    A real but small hormonal signal in trials of stressed or training men. If your testosterone is genuinely low on repeat morning testing, this is not the answer — see an endocrinologist.

    How It Works

    The proposed route runs through the stress axis. Chronically elevated cortisol suppresses gonadotropin-releasing hormone and, downstream, luteinising hormone and testicular testosterone production. By lowering cortisol, ashwagandha may partially release that suppression, which fits the observation that increases are largest in stressed or overtrained men. Trials have also reported increases in luteinising hormone and dehydroepiandrosterone, and improvements in sperm concentration and motility in men with infertility, alongside reduced markers of oxidative stress in seminal plasma.

    What the studies say

    Several randomized placebo-controlled trials have measured testosterone as an outcome. In resistance-training men taking 600 mg of KSM-66 daily for eight weeks, total testosterone rose meaningfully more than placebo alongside greater gains in strength and muscle size. In overweight men aged 40-70 with mild fatigue, 21-day supplementation increased DHEA-S substantially and testosterone modestly. Trials in men with infertility have reported improvements in sperm concentration, motility and testosterone. A systematic review of ashwagandha and male reproductive parameters concluded that the direction of effect is consistent but that sample sizes are small, populations differ widely, and the absolute changes in testosterone are modest — generally in the range of 10-15% from baseline, keeping men within the normal reference range rather than correcting a deficiency. No trial has established that ashwagandha treats clinical hypogonadism, and none should be read that way.

    Safety

    Caveats

    Low testosterone confirmed on two morning blood tests is a medical finding that deserves a workup, not a supplement — pituitary, testicular and metabolic causes all need excluding. The trial increases are modest and were mostly measured in men who were not hypogonadal. Ashwagandha can raise thyroid hormones, has rare associations with liver injury, and may stimulate immune activity, all of which apply here.

    Less likely to help if

    Men with primary or secondary hypogonadism from a structural or genetic cause, and men whose testosterone is already mid-normal, are unlikely to see clinically meaningful change.
