Ashwagandha produces modest increases in total testosterone in men in several small randomized trials, typically 10-15% and within the normal range. It is not a treatment for clinically diagnosed hypogonadism.

The ashwagandha group showed significantly greater increases in bench press and leg extension strength and larger reductions in exercise-induced muscle damage than placebo.

Ashwagandha increased salivary DHEA-S by 18% and testosterone by 14.7% versus placebo, but produced no significant improvement in self-reported fatigue, vigour or sexual well-being.
Safety
Caveats
Low testosterone confirmed on two morning blood tests is a medical finding that deserves a workup, not a supplement — pituitary, testicular and metabolic causes all need excluding. The trial increases are modest and were mostly measured in men who were not hypogonadal. Ashwagandha can raise thyroid hormones, has rare associations with liver injury, and may stimulate immune activity, all of which apply here.
