Overview
Low testosterone, or male hypogonadism, is a clinical diagnosis requiring two elements: symptoms consistent with androgen deficiency, and low total testosterone measured on at least two separate early-morning samples. A single reading is inadequate — testosterone follows a strong diurnal rhythm, peaking in the early morning, and varies day to day. The distinction between primary and secondary hypogonadism drives the workup. Primary hypogonadism originates in the testes and is marked by low testosterone with elevated LH and FSH. Secondary hypogonadism originates in the pituitary or hypothalamus, with low or inappropriately normal gonadotropins, and requires assessment of prolactin, iron studies and sometimes pituitary imaging. The most common contemporary pattern is functional secondary hypogonadism associated with obesity, metabolic syndrome, obstructive sleep apnoea, opioid use or chronic illness. This matters practically, because it is often partly reversible: substantial weight loss, treating sleep apnoea, and reducing opioid exposure all raise testosterone without any hormone being prescribed. Prevalence estimates vary widely with the threshold used, but symptomatic hypogonadism affects a few percent of middle-aged men and rises with age and with obesity. Age-related decline is real but gradual, roughly 1% a year after the age of 30, and most healthy older men remain within the normal range. Supplements have a limited role. Zinc and vitamin D correct deficiencies where those exist. Ashwagandha produces increases of roughly 10-15% in trials, mostly in men who were not hypogonadal to begin with. None of this treats a confirmed clinical deficiency.

Root Causes
Testosterone production depends on an intact hypothalamic-pituitary-gonadal axis. In functional secondary hypogonadism, the commonest pattern today, adipose tissue aromatase converts testosterone to oestradiol, which suppresses gonadotropin release, while inflammatory cytokines and the disrupted sleep of obstructive sleep apnoea further suppress the axis. The result is a self-reinforcing loop in which low testosterone promotes fat gain and fat gain lowers testosterone — one reason weight loss can be genuinely disease-modifying here.

When to See a Doctor
See a clinician for persistent low libido, erectile dysfunction, unexplained fatigue with loss of muscle mass, or infertility. Urgent assessment is warranted for visual field changes or headaches alongside these symptoms, which may indicate a pituitary lesion. Do not start testosterone based on an online test or a single reading.

A single decent trial is intriguing but not enough — and product purity matters more here than in almost any other supplement.
D-Aspartic Acid
DAA may bump testosterone for about two weeks in sedentary men, then levels return to baseline; in trained men it does nothing, and higher doses may backfire.
Tribulus Terrestris
This is the clearest negative in the testosterone-booster category — the marketing claim has been tested repeatedly and failed.
Tongkat Ali
Tongkat ali may raise testosterone in men with low baseline levels or high stress, but trials are small and often industry-linked.
Vitamin D
The association between vitamin D status and testosterone is consistent in observational data, but randomised trials are conflicting. Benefit appears limited to men who are genuinely vitamin D deficient.
Zinc
Zinc restores testosterone in men who are deficient and does essentially nothing in men who are not. Worth checking if you are vegetarian, a heavy drinker or have gut malabsorption; otherwise this is not the lever.
Magnesium
A small trial in athletes found magnesium raised free and total testosterone, with the larger rise in those also exercising. Evidence is limited to a handful of small studies and observational correlations, so effects are probably modest and largest when status is low.
Ashwagandha
A real but small hormonal signal in trials of stressed or training men. If your testosterone is genuinely low on repeat morning testing, this is not the answer — see an endocrinologist.

The dietary lever that matters most is achieving and maintaining a healthy body composition, since adipose tissue drives the aromatisation and inflammation that suppress the axis. Very low fat diets have been associated with modestly lower testosterone in some studies, so severe fat restriction is not advisable. Adequate zinc, vitamin D and overall energy intake support normal production; chronic energy deficiency, common in endurance athletes, suppresses it.
