    Mixed evidence

    Zinc restores testosterone in men who are deficient and does essentially nothing in men who are not. Worth checking if you are vegetarian, a heavy drinker or have gut malabsorption; otherwise this is not the lever.

    How It Works

    Zinc is a cofactor for enzymes in the steroidogenic pathway and is required for normal Leydig cell function and pituitary gonadotropin release. It also inhibits aromatase, the enzyme converting testosterone to oestradiol, which is the basis for claims about free testosterone. Experimental zinc restriction in healthy young men produces a marked fall in serum testosterone within twenty weeks, and repletion reverses it — a clean demonstration that the relationship is real but conditional on status.

    Evidence

    What the studies say

    The foundational work is Prasad's dietary restriction and repletion studies, which showed testosterone falling under zinc restriction and recovering with supplementation, including in marginally deficient older men. Later trials in men with adequate zinc status, including athletes, have generally shown no significant change in total or free testosterone. A 2023 review of the area concluded the effect is confined to correcting deficiency. Some evidence supports zinc for sperm parameters in subfertile men, which is a different endpoint and modestly better supported.

    Safety

    Caveats

    Do not stack this above 40 mg daily long-term chasing a bigger effect — you will induce copper deficiency, which causes anaemia and neurological symptoms. Confirm low testosterone on two morning samples before treating it at all. The largest randomised trial of zinc for male reproductive endpoints (FAZST, n=2,370) found no improvement in semen quality or live birth versus placebo; positive findings come mainly from small studies in zinc-deficient men.

    Less likely to help if

    Men with adequate zinc intake, which is most men eating meat regularly. Also ineffective where the cause is primary testicular failure, pituitary disease, obesity-driven aromatisation or opioid use.
