    Mixed evidence

    One of the few genuinely useful zinc applications. Lozenges started within 24 hours of the first symptom shorten a cold by roughly a day. The conditions are strict — right form, right dose, right timing — and most people get at least one of them wrong.

    How It Works

    The action appears to be largely local. Ionic zinc released in the oropharynx interferes with rhinovirus attachment to ICAM-1 receptors and with viral capsid assembly, while reducing local inflammatory mediator release. This is why the lozenge has to dissolve in the mouth: swallowing a capsule delivers zinc systemically and bypasses the site where the virus is replicating.

    Evidence

    What the studies say

    Meta-analyses of randomised trials, including Hemila's repeated analyses of zinc acetate lozenge studies, converge on a reduction in cold duration of roughly one to two days, or about 30% in relative terms, when daily elemental zinc is at least 75 mg. Trials using lower doses, zinc gluconate with confounding excipients such as citric acid that bind the zinc ion, or delayed initiation, have generally been null. A 2021 systematic review in BMJ Open found reduced symptom duration but noted the heterogeneity across formulations. Evidence for zinc as a preventive taken year-round is much weaker.

    Safety

    Caveats

    Metallic taste and nausea are common at these doses and are the main reason people stop. Start within 24 hours or do not bother. This is a short-course intervention only — 75 mg daily for weeks will cause copper deficiency. Cold-duration benefit is seen in adults but not children, heterogeneity across trials is very high, and a randomised trial of high-dose zinc gluconate in outpatient viral infection found no benefit over usual care.

    Less likely to help if

    Anyone starting after about 48 hours of symptoms. Also ineffective for influenza or COVID-19, where the rhinovirus mechanism does not apply, and for bacterial infections.
