    Legitimate as a stress-reduction aid in otherwise healthy people; it cannot fix adrenal dysfunction because that condition, as marketed, does not exist — and genuine Addison\u0027s must be excluded first.

    How It Works

    Withanolides modulate the HPA axis and GABAergic signaling; trials show reduced perceived stress and modestly lower morning cortisol in stressed but otherwise healthy adults.

    Evidence

    What the studies say

    Randomized trials (Chandrasekhar 2012 and subsequent replications, plus meta-analyses 2021–2022) show ashwagandha extracts reduce perceived stress scale scores and modestly lower serum cortisol versus placebo over 8 weeks in chronically stressed adults. This supports use for stress and sleep quality, but says nothing about adrenal pathology — no trial has enrolled patients with measured adrenal dysfunction, because no such validated entity exists outside true adrenal insufficiency. Practical caution: fatigue with weight loss, hyperpigmentation, or orthostatic dizziness needs an 8 AM cortisol and ACTH before any adaptogen trial, and rare liver injury has been reported with ashwagandha.

    Less likely to help if

    People with true Addison's disease or secondary adrenal insufficiency, and those whose fatigue is from sleep apnoea, iron deficiency or hypothyroidism.
