    Artichoke Extract for Cholesterol Management

    Artichoke leaf extract lowers total and LDL cholesterol by roughly 4-6% at doses of 1,280 mg/day or more over 8-12 weeks. The effect is consistent across trials but modest — a legitimate dietary adjunct, not a replacement for statin therapy in anyone at meaningful cardiovascular risk.

    Overview

    Artichoke leaf extract is one of the better-studied botanicals for lipid management, though the evidence base is modest in size. Randomised trials in adults with mild to moderate hypercholesterolaemia report reductions in total cholesterol of roughly 0.3 to 0.6 mmol/L over 8 to 12 weeks, with smaller and less consistent changes in LDL. The realistic framing is adjunctive. Artichoke is not a statin substitute, and nobody has shown that it changes cardiovascular events. It is a reasonable option for someone with borderline numbers who is already working on diet, or as an add-on where a small additional shift is useful.
    Products vary widely. Trials generally used standardised dry leaf extracts dosed at 1280 to 1920 mg daily, often expressed against caffeoylquinic acid or cynarin content. Culinary artichoke and artichoke juice are not equivalent to the extracts studied.

    A Cochrane review of randomised trials found small but consistent total-cholesterol reductions with standardised artichoke leaf extract. Trials are short, small, and heterogeneous in preparation, and no outcome data exist.

    How It Works

    The main proposed mechanism is inhibition of hepatic HMG-CoA reductase activity by luteolin and its glycosides, the same enzyme statins target, but with far weaker affinity. This would modestly reduce endogenous cholesterol synthesis rather than block it.
    A second route is bile. Artichoke is choleretic: cynarin and caffeoylquinic acids increase bile flow, which increases faecal loss of cholesterol-derived bile acids and forces the liver to draw on circulating cholesterol to replace them. This is also why artichoke has a traditional reputation for easing fatty-meal dyspepsia. Antioxidant effects on LDL oxidation and improvements in endothelial function have been reported in small studies, but these are secondary observations rather than the basis for the lipid effect.

    Dosing & Protocol

    Use a standardised dry leaf extract and give it time. Lipid changes are typically measured at 8 to 12 weeks, so a retest earlier than eight weeks will usually be uninformative. Split dosing with meals both improves tolerability and matches the bile-related mechanism.

    Safety

    Artichoke leaf extract is well tolerated in trials. The most common complaints are mild and gastrointestinal: flatulence, loose stools and transient cramping, usually settling within the first couple of weeks. Allergic reactions occur in people sensitised to the Asteraceae family, which includes ragweed, chrysanthemum and daisy.

    Avoid with bile duct obstruction

    Because artichoke stimulates bile flow, it should be avoided in bile duct obstruction and used only with medical advice in gallstone disease. Safety in pregnancy and breastfeeding has not been established. Do not replace prescribed lipid-lowering therapy without speaking to your clinician.
