    Overview

    Cynara scolymus leaf, not the edible heart, is the medicinal part, and cynarin and luteolin are the compounds that matter. The clearest clinical use is functional dyspepsia: randomised trials show reductions in bloating, early satiety, nausea and post-meal discomfort, likely because bile flow is a rate-limiting step in fat digestion and many people with dyspepsia have sluggish gallbladder emptying. A Cochrane review of artichoke for hypercholesterolaemia found modest but consistent total and LDL cholesterol reductions, driven by luteolin's inhibition of HMG-CoA reductase and increased biliary cholesterol excretion — real, but a fraction of what a statin delivers. There is also reasonable evidence in NAFLD, where trials report reduced liver enzymes and improved ultrasound findings at 600 mg/day of extract. The important safety boundary is biliary obstruction: increasing bile flow against an obstructed duct or an existing stone is exactly the wrong intervention, so gallstone disease must be excluded first.
