    How It Works

    Puerarin and daidzein influence lipid metabolism in rodent studies, including reduced LDL oxidation and improved cholesterol handling, and daidzein is metabolized to equol in some people, a more potent estrogenic compound. Whether these mechanisms lower cholesterol in humans at achievable doses is unknown.

    Evidence is insufficient. Kudzu root contains isoflavones including puerarin, daidzein and genistein, and rodent studies report reductions in total and LDL cholesterol along with anti-inflammatory and antioxidant effects, with plausible mechanisms via oestrogen receptor signalling and lipid metabolism. Puerarin has also been studied by injection in Chinese cardiovascular practice. But there are no adequate randomised controlled trials of oral kudzu root for cholesterol in humans - the human research on kudzu concerns reducing alcohol intake, where small trials do show an effect, and menopausal symptoms. Extrapolating a rodent lipid finding to a clinical recommendation is not justified.

    What the studies say

    Kudzu research in humans has focused almost entirely on alcohol-intake reduction, where small trials show a genuine signal. For cholesterol, the evidence is limited to animal and cell studies; no randomized human trial has tested kudzu extract for lipid endpoints. People looking to manage cholesterol have far better-supported options, including dietary change, psyllium, and plant sterols.

    Caveats

    Raised cholesterol is a major modifiable cardiovascular risk factor with treatments of proven mortality benefit - statins and ezetimibe - and it should not be managed with an unproven botanical. Do not stop or reduce prescribed lipid-lowering medication. Safety: kudzu isoflavones are phytoestrogens with oestrogen receptor activity, so avoid in hormone-sensitive cancers including breast, ovarian and uterine, and in endometriosis or uterine fibroids, without oncology advice. Kudzu may interact with tamoxifen and hormonal therapies, and with methotrexate. It may lower blood glucose - monitor if taking insulin or sulfonylureas - and may add to the effects of antihypertensives. Case reports of liver injury exist with kudzu-containing preparations, so avoid in liver disease and stop if jaundice, dark urine or right upper quadrant pain develops. There is no established upper intake level and no long-term safety data. Avoid in pregnancy and breastfeeding, where the phytoestrogen content makes safety particularly uncertain, and in children.
