    Overview

    Non-alcoholic steatohepatitis, increasingly termed metabolic dysfunction-associated steatohepatitis, is the inflammatory subset of fatty liver disease in which hepatocyte ballooning and lobular inflammation accompany fat accumulation. This distinction matters enormously: simple steatosis carries limited hepatic risk, while steatohepatitis progresses to fibrosis, cirrhosis and hepatocellular carcinoma. Fibrosis stage, not the degree of fat or inflammation, is the strongest predictor of liver-related and overall mortality. Diagnosis has moved away from biopsy for most people. Non-invasive scores such as FIB-4 stratify risk cheaply, and transient elastography quantifies stiffness, with biopsy reserved for uncertain cases. Because the disease is metabolic in origin, the interventions with the largest effect are metabolic. Weight loss is dose-dependent and well characterised: around 5% reduces steatosis, 7% resolves steatohepatitis in a substantial proportion, and 10% improves fibrosis. Exercise reduces liver fat even without weight change. GLP-1 receptor agonists and resmetirom have shifted the pharmacological landscape substantially. Among supplements, vitamin E stands out as genuinely evidence-based. The PIVENS trial showed 800 IU daily improved steatohepatitis histology in non-diabetic adults without cirrhosis, which is why guidelines list it. Omega-3 reduces liver fat and triglycerides but does not resolve inflammation. Coffee, notably, associates consistently with less fibrosis.

    When to See a Doctor

    See a doctor for persistently abnormal liver enzymes, or fatty liver found incidentally on imaging, so that fibrosis can be staged. Seek urgent care for jaundice, abdominal swelling, vomiting blood, or confusion, which suggest decompensated cirrhosis.
