Fibromyalgia is a real disorder of pain processing, not a diagnosis of exclusion by default. Graded exercise, sleep repair and cognitive approaches outperform every supplement studied so far.
Overview
Fibromyalgia is characterised by widespread pain lasting more than three months, unrefreshing sleep, fatigue and cognitive fog. The underlying problem is central sensitisation: the nervous system amplifies normal signals, so pain is generated by processing rather than by tissue damage. That explains why imaging and inflammatory markers come back normal and why anti-inflammatory strategies underperform. Diagnosis is positive and clinical, using the widespread pain index and symptom severity score, though thyroid disease, inflammatory arthritis, vitamin D deficiency, anemia and sleep apnea should be excluded because they mimic or worsen it. The interventions with the strongest evidence are unglamorous: graded aerobic exercise started well below the level that provokes a flare and increased slowly, treatment of the sleep disturbance, cognitive behavioural therapy for pain, and where needed duloxetine, milnacipran or pregabalin. Supplement evidence is modest. Magnesium, vitamin D correction when deficient, coenzyme Q10 and low-dose 5-HTP have small supportive trials, none large or replicated enough to be a foundation. Pacing, not pushing, is the practical principle that most changes outcomes.

- TSH to exclude thyroid disease
- Full blood count and ferritin
- CRP and ESR to exclude inflammatory arthritis
- Vitamin D level
- Sleep apnea screening when snoring or daytime sleepiness is present
When to See a Doctor
See a clinician for widespread pain lasting more than three months, especially with fatigue and sleep disturbance. Joint swelling, fever, rash, weight loss or new weakness point away from fibromyalgia and need prompt evaluation.
Supplements Studied For This
Magnesium
Small trials of magnesium, mostly as magnesium malate combined with malic acid, show inconsistent effects on fibromyalgia pain.
CoQ10
Several small randomised trials from a single research group report reduced pain and fatigue with 300 mg of CoQ10 daily, but replication is limited.
5-HTP
Older trials suggest 5-HTP reduces fibromyalgia pain and improves sleep, but the studies predate modern criteria and serotonergic interaction risk needs care.

Suggested Pattern
No specific diet is proven. A whole-food, largely plant-forward pattern with adequate protein supports exercise tolerance and weight management, both of which affect symptom burden.
