    Several small randomised trials from a single research group report reduced pain and fatigue with 300 mg of CoQ10 daily, but replication is limited.

    How It Works

    Mitochondrial dysfunction and oxidative stress have been documented in blood mononuclear cells of people with fibromyalgia, with reduced CoQ10 levels reported. Supplementation is proposed to restore mitochondrial bioenergetics and lower oxidative markers, reducing symptom burden.

    Evidence

    What the studies say

    Randomised, placebo-controlled trials using 300 mg daily for around 40 days reported significant reductions in pain, fatigue and tender point count, alongside improvements in mitochondrial and oxidative stress markers. The results are internally consistent but come predominantly from one Spanish research group with samples in the tens, and independent replication at scale has not followed. A systematic review concluded the evidence was encouraging but insufficient to recommend routine use. CoQ10 is well tolerated, which lowers the cost of a structured trial, but it should sit alongside graded exercise, sleep treatment and cognitive approaches rather than replace them.

    As compared with the placebo groups evaluated in each RCT, the CoQ10 group showed a statistically significant reduction in fatigue scores (Hedges g = -0.398, 95% confidence interval = -0.641 to -0.155, p = 0.001).

    Safety

    Caveats

    Evidence concentrated in one research group with small samples. CoQ10 can reduce warfarin effect and may modestly lower blood pressure. The cost of 300 mg daily is not trivial for an unconfirmed benefit.

    Less likely to help if

    People whose main driver is unmanaged sleep apnea, depression or deconditioning will not improve on CoQ10 alone.
