Eritadenine, a compound found in shiitake, lowers cholesterol substantially in rats by altering phospholipid metabolism and hepatic lipid handling, and this is the entire basis of the claim. Human data are sparse: a few small and old studies, some in young women eating fresh shiitake, reported modest cholesterol reductions, while others found nothing. No adequately powered modern randomised controlled trial exists, the eritadenine content of commercial products is unknown and unstandardised, and no trial has examined cardiovascular outcomes.

    What the studies say

    Rodent studies consistently show meaningful reductions in serum cholesterol with dietary shiitake or isolated eritadenine, an effect strong enough to have driven decades of interest. Human confirmation is scarce: a small study of young women eating 10g dried shiitake daily reported modest total cholesterol reductions, while other feeding studies found no significant lipid change. No adequately powered randomized trial has tested a standardized shiitake extract against placebo for LDL. Beta-glucan-mediated bile-acid binding is plausible but weaker per gram than oat or barley beta-glucan, which do have approved health claims. Shiitake remains valuable as a low-calorie, nutrient-dense food, but the lipid claim outruns the human evidence.

    Caveats

    Do not substitute shiitake for statins or other prescribed lipid therapy. Raw or undercooked mushrooms can cause a flagellate dermatitis.
