However, when independent groups tested Cuban and non-Cuban policosanol using rigorous methods, lipid effects vanished: a 2006 JAMA trial (Berthold et al.) found no effect on LDL, HDL, or triglycerides across multiple doses. Hypotheses for the discrepancy include publication bias, product differences, and diet interactions, but none are proven. Small trials suggesting antiplatelet effects exist but are insufficient for clinical claims. Policosanol is safe and cheap, but consumers seeking LDL reduction should look to red yeast rice (with caveats), berberine, psyllium, or conventional therapy instead.
