    Omega-3 Fatty Acids for Chronic Inflammation

    Omega-3 measurably lowers inflammatory markers such as CRP, IL-6 and TNF-alpha in randomized trials, with larger effects at doses above 2 g/day of EPA+DHA and in people who start with elevated inflammation.

    Marine omega-3s are the best-characterised nutritional anti-inflammatory available, and the mechanistic account is unusually complete: we know which mediators they generate, how membrane composition changes, and roughly how long it takes. Trials at 2 to 3 g daily of combined EPA and DHA reduce C-reactive protein, interleukin-6 and TNF-alpha, with the largest effects in people who start with elevated inflammatory markers. The caveat is proportion. These are meaningful biochemical shifts, not drug-scale suppression, and they matter most as part of a broader pattern that includes diet, sleep, activity and body composition.
    Standard fish oil capsules rarely provide the doses used in inflammation trials. A typical 1000 mg capsule contains around 300 mg of EPA plus DHA, so reaching 2 to 3 g means concentrated products or several capsules — the single most common reason people conclude fish oil did nothing for them.

    How It Works

    EPA and DHA displace arachidonic acid from membrane phospholipids, so when phospholipase A2 releases substrate during an inflammatory stimulus, the eicosanoids produced are the less inflammatory series-3 prostaglandins and series-5 leukotrienes rather than the potent series-2 and series-4 mediators derived from arachidonic acid.
    The more interesting mechanism is active resolution. EPA and DHA are precursors to specialised pro-resolving mediators — resolvins, protectins and maresins — which do not merely suppress inflammation but actively terminate it, promoting macrophage clearance of apoptotic neutrophils and tissue repair. This is a distinct pharmacology from NSAIDs, which block mediator synthesis without driving resolution. Omega-3s also signal through GPR120 to inhibit NF-kappaB activation and dampen NLRP3 inflammasome assembly, reducing IL-1beta and IL-18 output. Membrane incorporation takes months, which is why anti-inflammatory effects build slowly.

    Dosing & Protocol

    Dose by EPA plus DHA content, not total fish oil weight, and take with a fat-containing meal. Triglyceride and re-esterified triglyceride forms absorb somewhat better than ethyl esters. An omega-3 index above 8% is the usual target if you want an objective measure of adequacy.

    Safety

    Fishy burps, reflux and loose stools are the routine complaints and are largely managed by taking capsules with food, splitting doses, or freezing them. Oxidised oil is worth watching for: a strongly fishy smell on opening indicates rancidity, and oxidised omega-3s are pro-inflammatory rather than anti-inflammatory.

    Bleeding, rhythm and chronic inflammation red flags

    Omega-3s have mild antiplatelet activity: stop about 7 days before surgery and take advice before combining with anticoagulants. High-dose omega-3 at 3 to 4 g/day has been linked to a small increase in atrial fibrillation. Persistent unexplained inflammation with fever, weight loss, night sweats, joint swelling or abnormal blood counts requires medical investigation — supplements should not delay a diagnosis of autoimmune or malignant disease.
