TL;DR
Chronic low-grade inflammation is a persistent, subclinical immune activation linked to cardiometabolic and neurodegenerative disease. It is measured by hs-CRP and driven mostly by adiposity, inactivity, poor sleep and smoking.
Overview
Acute inflammation is a healthy, self-limiting response to injury or infection. Chronic low-grade inflammation is something different: a persistent, low-amplitude activation of the innate immune system without an acute trigger, sustained over years. It produces no redness or swelling and is detectable only through biomarkers, principally high-sensitivity C-reactive protein. Its clinical importance comes from its association with disease. Elevated hs-CRP predicts cardiovascular events independently of cholesterol, and the CANTOS trial demonstrated that directly inhibiting interleukin-1 beta reduced cardiovascular events without altering lipids — the strongest evidence that inflammation is causal rather than merely correlated. Similar associations exist for type 2 diabetes, some cancers and cognitive decline. The dominant driver in most people is visceral adipose tissue, which is metabolically active and secretes interleukin-6 and tumour necrosis factor alpha. Physical inactivity, poor sleep, smoking, periodontal disease and chronic psychological stress all contribute independently. This is where the marketing runs well ahead of the science. Anti-inflammatory supplements are sold aggressively, and while omega-3 and curcumin do produce measurable reductions in inflammatory markers, the reductions are modest and the evidence that lowering a marker improves outcomes is limited to specific settings. Weight loss, exercise and smoking cessation move these markers more than any supplement, and their benefit on hard outcomes is established rather than inferred. One practical caution: hs-CRP rises sharply with any acute infection, so a raised value should be rechecked at least two weeks after any illness before it is interpreted.

Root Causes
Visceral adipose tissue behaves as an endocrine organ, releasing interleukin-6 and tumour necrosis factor alpha and recruiting macrophages that shift toward a pro-inflammatory phenotype. Interleukin-6 drives hepatic C-reactive protein production, which is why hs-CRP tracks adiposity so closely. Parallel contributors include innate immune priming by metabolic signals, translocation of bacterial lipopolysaccharide across a compromised gut barrier, and cellular senescence with its associated secretory phenotype, which is thought to underlie much age-related inflammation.

When to See a Doctor
See a clinician for persistently raised inflammatory markers on repeat testing, for joint swelling, morning stiffness lasting more than an hour, unexplained fever, night sweats or weight loss — these suggest an inflammatory or autoimmune disease rather than low-grade inflammation and need proper investigation.
Supplements Studied For This
Serrapeptase
The historical evidence does not survive modern scrutiny, and a regulator has already acted on that — we would not recommend it for inflammation.
Curcumin
Curcumin reliably lowers CRP and IL-6 in people who start with them raised. Whether that translates into better long-term outcomes is untested, so treat it as a marker-moving intervention rather than a proven disease preventer.
Omega-3 Fatty Acids
A well-supported intervention for lowering measured inflammatory markers. Whether that translates to how you feel depends heavily on what is driving the inflammation.
Quercetin
Quercetin lowers CRP in meta-analysis, particularly at doses above 500mg daily and in people with elevated baseline inflammation. Bioavailability is poor and translation of biomarker change into clinical benefit is unproven.

Suggested Pattern
The Mediterranean pattern has the most consistent evidence for lower inflammatory markers — oily fish, olive oil, nuts, legumes, vegetables and whole grains, with limited red and processed meat. Fibre matters through short-chain fatty acid production by gut bacteria. Reducing ultra-processed food and added sugar lowers markers, though much of that effect runs through weight loss. Individual anti-inflammatory foods deserve less attention than the overall pattern and the resulting body composition.

Who It Affects
Depending on the threshold used, roughly a quarter to a third of adults have hs-CRP above 3 mg/L, the level conventionally regarded as higher cardiovascular risk.
Markers rise with age and with body mass index, and are higher in smokers, in people with metabolic syndrome, and in those with obstructive sleep apnoea. Women have slightly higher average hs-CRP than men at equivalent body composition.
