    Omega-3 Fatty Acids for Joint Pain

    Omega-3 reduces morning stiffness, tender joint counts and NSAID use in rheumatoid arthritis at doses around 2.7 g/day of EPA+DHA. Evidence in osteoarthritis is considerably weaker.

    Overview

    For joint pain, omega-3s have one of the more useful evidence bases in the supplement world, but the benefit is unevenly distributed. Meta-analysis of randomised trials of marine oil supplements for arthritis pain shows reductions in pain intensity and in NSAID requirement, with the clearest effects in inflammatory arthritis — rheumatoid arthritis in particular — and much weaker effects in osteoarthritis. That distinction follows directly from mechanism: omega-3s modulate inflammatory mediators, so they help most where inflammation is driving the pain rather than mechanical cartilage loss.
    The NSAID-sparing effect is arguably the most practical finding. Being able to reduce chronic NSAID use meaningfully lowers gastrointestinal and cardiovascular risk over years, even if the direct analgesic effect is modest. Expect 12 weeks before judging, and expect to need 2.7 g or more of combined EPA and DHA daily — the threshold most consistently associated with benefit in inflammatory arthritis.

    How It Works

    In inflamed synovium, arachidonic acid released from membrane phospholipids feeds cyclooxygenase and 5-lipoxygenase, producing prostaglandin E2 and leukotriene B4 — the mediators most directly responsible for joint pain, swelling and neutrophil recruitment. EPA and DHA displace arachidonic acid from those membranes, reducing the substrate available and shifting output toward less inflammatory series-3 and series-5 mediators.
    Resolvins and protectins derived from EPA and DHA actively drive resolution in synovial tissue, promoting clearance of inflammatory cells rather than simply blocking mediator production. Omega-3s also reduce cartilage-degrading matrix metalloproteinase expression and aggrecanase activity in chondrocyte models, which is the theoretical basis for a structural effect in osteoarthritis that human trials have not convincingly demonstrated. The requirement for months of dosing reflects the pace of membrane phospholipid turnover in synovial and immune cells.

    Dosing & Protocol

    This is a high-dose indication. Read EPA plus DHA per capsule and use a concentrated product, or you will be swallowing eight or more standard capsules a day. Take with meals containing fat, and choose a product with a recent oxidation certificate.

    Safety

    At the doses needed for joint pain, gastrointestinal effects are more noticeable: fishy burps, reflux and loose stools. Splitting the dose across meals, freezing capsules, or using enteric-coated products handles most of it. Rancid oil should be returned rather than tolerated.

    High doses need extra care

    At 3 g/day and above, the antiplatelet effect becomes clinically relevant — stop about 7 days before surgery, and get prescriber approval before combining with anticoagulants, which is common in this population. High-dose omega-3 has been linked to a small increase in atrial fibrillation. Do not stop disease-modifying antirheumatic drugs or biologics; omega-3 is an adjunct. New joint swelling, redness, fever, or morning stiffness lasting over an hour needs rheumatology assessment.
