- Evidence is insufficient: no controlled trial has shown that omega-3 lowers thyroid peroxidase antibodies, preserves thyroid function or reduces levothyroxine requirement in Hashimoto thyroiditis. The doses studied for general inflammation, 1000-2000 mg/day combined EPA+DHA, produced no thyroid-specific outcomes. Selenium has more (though still limited) antibody data, and levothyroxine is the treatment for the resulting hypothyroidism.

Caveats
Do not adjust or stop levothyroxine - untreated hypothyroidism carries cardiac and, in pregnancy, fetal risk. Safety ceiling: below 3 g/day EPA+DHA without supervision; higher increases bleeding risk with anticoagulants and antiplatelets, and should be stopped before surgery. Separate any supplement containing iron or calcium from levothyroxine by four hours. Standard omega-3 doses are acceptable in pregnancy; avoid cod liver oil because of vitamin A.
