    Mixed evidence

    Biologically plausible and safe, but the randomized evidence does not support omega-3 as a way to prevent cognitive decline in people who are already declining. Best framed as long-term dietary quality rather than treatment.

    How It Works

    DHA is the dominant polyunsaturated fatty acid in neuronal membranes and is concentrated in synaptic terminals, where it influences membrane fluidity, receptor function and neurotransmitter release. It supports synaptic plasticity, promotes production of the neuroprotective metabolite neuroprotectin D1, and contributes to the anti-inflammatory environment of the brain. Omega-3 also improves cerebral blood flow and reduces the vascular contribution to cognitive impairment.

    Evidence

    What the studies say

    The observational literature is encouraging and consistent. Cohort studies repeatedly find that higher fish consumption and higher plasma or red-cell DHA associate with less cognitive decline, smaller white matter lesion burden and larger hippocampal volume over follow-up. The randomized evidence is much less encouraging. Large trials of omega-3 supplementation in older adults, including cognitive substudies of major cardiovascular trials, have generally found no meaningful effect on cognitive test scores over three to five years. Trials in established Alzheimer disease have been clearly negative. A Cochrane review of omega-3 for cognitive decline and dementia concluded that supplementation for six to forty months provides no benefit for cognitive function in cognitively healthy older adults. Two interpretations compete. One is confounding: people who eat fish differ in many ways that also protect cognition. The other is timing: neuronal membrane composition may be laid down over decades, so intervening in the seventh decade for three years may simply be too late and too brief. Some analyses suggest benefit is concentrated in APOE4 non-carriers and in those starting with the lowest omega-3 status, but these are subgroup findings and should be treated as hypotheses.

    Safety

    Caveats

    Do not present omega-3 as a treatment for dementia or mild cognitive impairment — the randomized trials in those populations are negative. Anyone with new or progressive memory problems needs a medical assessment, since reversible causes such as B12 deficiency, hypothyroidism, depression, sleep apnoea and medication effects are common and treatable.

    Less likely to help if

    People with established Alzheimer disease, and those who already have a high omega-3 index from regular fish intake, show no benefit in trials.
