Bacopa Monnieri for Cognitive Decline
Bacopa improves delayed word recall in older adults over 12 weeks, but no trial shows it slows genuine cognitive decline or dementia progression.

How It Works
Dosing & Protocol
Typical dose
- Recommended dose
- 300-450mg standardized extract daily (50% bacosides)
- Expected timeframe
- 8-12 weeks
Protocol
- form
- Extract standardised to 50% bacosides (CDRI 08 or BacoMind in most trials)
- timing
- With a meal containing fat - bacosides absorb better and stomach upset is much less likely
- duration
- 12 weeks minimum; nothing meaningful happens before 8-12 weeks
- titration
- Increase to 450 mg/day after 4 weeks if tolerated; taking it with food reduces the common gastrointestinal side effects
- measurement
- A delayed word-recall test at baseline and week 12, since that is the domain trials actually improved
- starting dose
- 300 mg standardised extract (50% bacosides) once daily with a fat-containing meal

What the studies say
Evaluating the effects of Bacopa monnieri on cognitive performance and sleep quality of patients with mild cognitive impairment: A triple-blinded, randomized, placebo-controlled trial
Delfan M, Kordestani-Moghaddam P, Gholami M +2 more
The results showed no statistically significant difference between two groups in all three time-points in overall cognitive performance score and its 6 parameters (P > 0.05).
Meta-analysis of randomized controlled trials on cognitive effects of Bacopa monnieri extract
Kongkeaw C, Dilokthornsakul P, Thanarangsarit P +2 more
Bacopa significantly improved speed of attention (Trail Making Test and choice reaction time) but most memory subtests were not significantly improved.
Safety
Caveats
The improvement is small and confined to delayed recall - attention and executive function do not reliably change, and no trial shows it slows dementia. Gastrointestinal side effects (cramping, nausea, loose stools) are common and dose-related; always take with food. Bacopa has cholinergic activity - caution with anticholinergics, cholinesterase inhibitors and in bradycardia, asthma and peptic ulcer disease. It may raise thyroid hormone levels. Sedating for some. Not established as safe in pregnancy or breastfeeding - avoid.
