    Omega-3 Fatty Acids for Arterial Flexibility

    Meta-analyses show omega-3 supplementation reduces pulse wave velocity and augmentation index, with a modest but reproducible effect.

    Overview

    Arterial flexibility is measured, not felt - and omega-3 is one of the few supplements with randomised trial evidence against a hard vascular measure, pulse wave velocity.
    Meta-analyses report reductions in pulse wave velocity and arterial augmentation index with EPA and DHA supplementation, typically at 1 to 3 g daily over three months or more. Arterial stiffness independently predicts cardiovascular events, so improvement on this measure is more meaningful than most surrogate endpoints. The effect is modest, larger in people who already have stiff arteries - older adults, hypertensives, people with diabetes - and unlikely to be noticeable in young, healthy vessels. It also does not reverse established atherosclerotic structural change.

    How It Works

    Arterial stiffness has two components: functional tone, set by endothelial nitric oxide and smooth muscle activity, and structural composition, set by the ratio of elastin to collagen and by advanced glycation cross-links. Supplements can influence the first substantially and the second barely at all.
    EPA and DHA incorporate into endothelial membranes, improving endothelial nitric oxide synthase function and reducing oxidative destruction of nitric oxide, which lowers resting smooth muscle tone. They also reduce vascular inflammation through NF-kB suppression and pro-resolving mediators, and lower triglycerides, which contributes indirectly. Because the mechanism is predominantly functional, benefits appear over weeks to months and plateau; they do not accumulate into structural remodelling of the arterial wall.

    Blood pressure control does more

    Treating hypertension, exercising aerobically and stopping smoking change arterial stiffness far more than supplementation. Omega-3 is an addition to those, not an alternative.

    Pooled analyses of randomised controlled trials report statistically significant reductions in pulse wave velocity and augmentation index with omega-3 supplementation, supported by consistent improvements in flow-mediated dilation and small reductions in blood pressure. Using validated haemodynamic measures rather than self-report is a genuine strength of this literature. Limitations include modest effect sizes, considerable heterogeneity in dose and duration, measurement variability in pulse wave velocity between devices and operators, frequent null results in young healthy cohorts, and no demonstration that the stiffness change translates into fewer cardiovascular events.

    Measured improvement, unproven outcomes

    Good evidence on validated stiffness measures. No trial has shown the change reduces heart attacks or strokes.

    Safety

    Omega-3s are well tolerated at these doses; reflux, fishy repeat and loose stools are the main issues and are reduced by dividing doses and taking them with food. Choose low-oxidation products, since oxidised oil is pro-oxidant in exactly the vascular tissue you are trying to protect.

    Arterial stiffness is a cardiovascular risk signal

    If you have raised blood pressure, chest pain, breathlessness or a strong family history, get a proper cardiovascular risk assessment rather than self-managing with supplements.

    Bleeding risk rises modestly above 3 g daily and matters for anyone on anticoagulants or approaching surgery. High-dose EPA formulations have also been associated with a small increase in atrial fibrillation risk in outcome trials, which is worth weighing if you have a history of palpitations or arrhythmia.
