    L-Citrulline for Arterial Flexibility

    Overview

    Arterial flexibility - how readily large arteries expand and recoil with each heartbeat - stiffens with age and predicts cardiovascular risk independently of blood pressure. L-citrulline is one of the more plausible nutritional interventions for it.
    Citrulline raises plasma arginine more effectively than arginine itself, supporting nitric oxide production and endothelium-dependent vasodilation. Trials have reported reductions in arterial stiffness measures such as pulse wave velocity and augmentation index, alongside modest blood pressure reductions. The honest framing is that the effect is real but small and measured mostly over weeks in small samples. No trial has shown that citrulline-driven improvements in stiffness translate into fewer cardiovascular events.

    How It Works

    Oral citrulline escapes intestinal and hepatic arginase, is taken up by the kidneys and converted to arginine, and therefore raises circulating arginine more efficiently and durably than an equivalent dose of arginine.
    That arginine feeds endothelial nitric oxide synthase. Nitric oxide diffuses to vascular smooth muscle, raises cyclic GMP and produces relaxation, which reduces arterial tone and improves the dynamic component of arterial compliance. The distinction that matters is between functional and structural stiffness. Nitric oxide can relax smooth muscle tone, which measurements pick up within weeks. It does not reverse the collagen deposition, elastin fragmentation and glycation that cause structural stiffening over decades - so a ceiling on the effect should be expected.

    Randomised trials and pooled analyses report that citrulline supplementation modestly lowers systolic and diastolic blood pressure and improves markers of endothelial function such as flow-mediated dilation, with several studies showing reductions in augmentation index or pulse wave velocity. Effects appear larger in people who start with hypertension or existing endothelial dysfunction. Limitations are consistent across this literature: small samples, durations of weeks rather than years, varied stiffness measures that are not directly comparable, and frequent industry funding. Improvements in a surrogate marker have not been linked to clinical outcomes here.

    Safety

    Citrulline is one of the better-tolerated amino acid supplements. Unlike arginine it rarely causes gastrointestinal upset even at several grams, and reported side effects are limited to mild bloating or stomach discomfort.

    Watch blood pressure if you are already medicated

    Citrulline lowers blood pressure modestly, which can add to antihypertensive therapy. Monitor readings when you start, and be alert to dizziness on standing.

    Long-term safety beyond a few months has not been established, and there is no adequate data in pregnancy or breastfeeding. People with kidney impairment should seek medical advice, since citrulline is converted to arginine in the kidneys and urea cycle handling may differ.
