Controlled studies show that colostrum at 20 g/day substantially attenuates the rise in intestinal permeability caused by strenuous exercise in the heat, and separate work shows it reduces the increase in gut permeability caused by short-term NSAID use — measured objectively by lactulose-mannitol ratios rather than by symptom questionnaires. This is a genuinely useful finding for endurance athletes and for people who need regular anti-inflammatories. The second reasonable claim is respiratory infection in athletes, where several trials report fewer upper respiratory episodes and fewer days of symptoms, likely mediated by secretory IgA. Beyond that the evidence thins rapidly. IGF-1 in colostrum is present but almost entirely digested, so anabolic claims rest on very little, and trials of colostrum for body composition in trained individuals are unimpressive. Practical constraints matter: effective doses in trials are 20-60 g/day, far above the one or two grams in a typical capsule, so cost becomes real. Colostrum is a dairy product and unsuitable for people with cow's milk protein allergy.
