    Overview

    Akkermansia muciniphila lives in the mucus layer of the gut and typically makes up 1-4% of the adult microbiome. Lower abundance is consistently observed in obesity, type 2 diabetes and inflammatory bowel disease, which is what put it on the map as a therapeutic candidate.

    The pivotal human data is a 2019 exploratory, randomized, double-blind, placebo-controlled trial in 32 insulin-resistant, overweight adults (Depommier et al., Nature Medicine). Three months of daily pasteurized A. muciniphila improved insulin sensitivity, modestly reduced insulinemia and total cholesterol, and produced a small drop in body weight versus placebo. Curiously, the pasteurized form outperformed the live form, likely because the surface protein Amuc_1100 survives pasteurization and drives TLR2 signalling.

    That is one small trial. Product quality varies enormously, and most consumer capsules are not the pasteurized strain used in the research. Treat this as promising, not established.
