    TL;DR

    Over-the-counter digestive enzymes help genuine pancreatic insufficiency very little — that needs prescription-strength PERT. For ordinary bloating after meals, enzyme supplements are mostly unnecessary.

    Why It Matters

    Exocrine pancreatic insufficiency (EPI) is a real diagnosis, caused by chronic pancreatitis, cystic fibrosis, pancreatic surgery or advanced diabetes. It presents with steatorrhoea (pale, greasy, floating stools), weight loss, and fat-soluble vitamin deficiency. It is treated with prescription pancreatic enzyme replacement therapy dosed in tens of thousands of lipase units per meal — orders of magnitude above supplement products. Self-treating suspected EPI with retail enzymes risks malnutrition.

    Optimization Protocol

    1) If you have greasy, floating stools with weight loss, get faecal elastase tested — do not self-treat. 2) Confirmed EPI requires prescription PERT titrated to meal fat content, plus fat-soluble vitamin repletion. 3) For ordinary post-meal bloating without EPI, address meal size, eating speed, alcohol and FODMAP load first. 4) Broad-spectrum OTC enzymes may modestly ease heaviness after very high-fat meals but do not correct malabsorption. 5) Lactase for lactose intolerance and alpha-galactosidase for legume gas are the two OTC enzymes with genuinely targeted evidence.
