How to Measure
Start with ALT, AST, GGT, ALP, bilirubin and albumin. Combine ALT, AST, platelets and age into a FIB-4 score to stratify fibrosis risk cheaply. Ultrasound detects steatosis but not mild fibrosis; transient elastography (FibroScan) measures stiffness non-invasively and MRI-PDFF quantifies fat precisely. Biopsy remains the reference standard but is rarely needed for initial assessment.

Resistance and aerobic exercise reduce liver fat even without weight change. Among supplements, vitamin E at 800 IU/day has trial support in non-diabetic biopsy-proven steatohepatitis but carries its own risk profile; betaine and choline support hepatic fat export; milk thistle has a long tradition and weak trial data; coffee is consistently associated with lower fibrosis risk in cohorts. Be careful with high-dose green tea extract and kava, both linked to hepatotoxicity.
