    TL;DR

    Galactagogue evidence is weak overall: fenugreek shows the most (mixed) human data; moringa and shatavari have preliminary support. Milk removal frequency — nursing/pumping more often — remains the proven driver of supply.

    Why It Matters

    Perceived low supply is the top reason for early weaning, yet true insufficiency is uncommon. Supply is demand-driven: effective, frequent milk removal is the primary lever. Fenugreek increased volume in several small trials (mechanism unknown; maple-syrup body odor is a common side effect) but can cause GI upset and, paradoxically, reduce supply in some women. Moringa showed increased prolactin and volume in small RCTs. Always rule out latch issues with a lactation consultant first.

    Track infant weight and diaper output — objective supply markers — rather than pump output alone.

    Optimization Protocol

    First: lactation consultant assessment of latch and transfer; nurse/pump 8–12x/day including overnight. Then consider: fenugreek 1,800–2,700 mg/day divided (1–2 week trial), moringa leaf 500 mg 2x/day, or shatavari 3 g/day. Hydration and adequate calories (+500 kcal/day while nursing). Avoid fenugreek with peanut/chickpea allergy or thyroid issues.
